Should you wish to switch to another health centre in Finland, you can use this form to notify of the matter. Please fill in the form, print it and sign. Deliver the form to your new health centre and provide a copy to your current health centre. More detailed instructions can be found below the form. You can also draft a written notification of another kind, if it contains all the necessary information. Please note that a separate form must be filled in for each family member.
Dear valued customer,
You are entitled to choose the health centre that treats you among all health centres in Finland. However, you can only choose one health centre at a time.
Should you wish to switch to another health centre, you must notify the current and the new health centre of the matter in writing. Please deliver this form to the health centre you would like to switch to or to the registry office of the municipality in which it operates. In addition, please provide a copy of the notice to your current health centre. The responsibility for treatment will be transferred to the health centre of your choice within three weeks of the receipt of the notice.
The notice applies to one person only. Therefore, a separate form must be filled in for each family member.
The change applies to all non-urgent health centre services, such as appointments with doctors, nurses and dentists, physiotherapy, assistive device services, and distribution of medical supplies. The change also applies to short-term in-patient care at a health centre. The change does not apply to student health care, long-term institutional care, or home care.
The notice is binding and effective until further notice. You cannot change your health centre again within one year. If you move to another municipality, the one-year restriction does not apply.
Consent regarding the disclosure of patient data
You may submit a consent or restriction regarding the disclosure of your patient data at a health care unit. A consent and restriction form can also be completed online via the My Kanta service, if the health care unit in question has taken the Patient Data Repository in use. The disclosure of patient data is governed by the Act on the Status and Rights of Patients (785/1992), Act on the Electronic Processing of Client Data in Social and Health Care (159/2007), and the Health Care Act (1326/2010).
